Addressing Health Inequalities:
1. COMMUNTY HEALTH CONNECTER
CNWLNHS funded our first full time health connector in December 2020. The connector is addressing the health inequalities faced by the hard to reach and disadvantaged communities from BAME. The service aims to support people with non-medical issues to access a range of community-based services through signposting and one to one support. It is a great asset to the community. Over 500 people has been supported in multiple problems including emergency food to mental health issues.We are trusted by the community now
2. Health Educators
From June 2021 to November 2021, we recruited 3 Health Educators in our locality with a partnership of four other voluntary sector organisation lead by Brent Carers to:
Promote, maintain and improve a healthy lifestyle to people and the wider community.
Assist Individuals and the Community to access the Health and Support that is suitable for them.
Raising Awareness on Health Inequalities and the impact it has on the Local community.
Program of engagement with individuals and community including health education events,workshops and presentations both face-to-face and digitally
To work as the link between the community and partners responsible for the delivery of the Brent Health Matters Program by delivering key health messages to and from the community.
They had 1400 engagements since June 2021 until 31 March 2022
3. Women’s Mental Health and Well-being Workshops
From February 2022, we have started the first set of 6 once a week thematic workshops and consultations for women from our community to understand mental issues and where to go for help. An average of 54 women attended in all the workshops and the number was increasing each time. Next will be starting after Ramadan in May and it will at the Unity Center in Church Road.
EMERGENCY
Supported 1540 cases and individuals’ benefits, housing, Health including mental health and bereavement.
6130 meals were delivered for 2 days a week, but we have exceptional cases who receive throughout the week for valuable reasons.
780 shopping baskets and collections for medications were delivered.
104 refugees and Asylum seekers in 3 different hotels in Brent has been assisted since November last year to register with the local GPs. We managed to provide to all of them translation and interpretation services to help them understand the GP and hospitals letters, text messages and telephone consultation. We also top their phones; winter clothes were provided to some of them and hot meals during the Ramadan both iftar and Suhur on every Friday.

Covid Awareness Campaigns
We actively participated Covid 19 awareness raising campaigns through videos
Vaccination Bus
Our staff has been in the frontline of vaccinations campaign until now and we all had vaccination.
We moved in partnerships as a way forward with the future and adapting the new normal. Health Educators Partnership service is one of the leads by Brent Carers Services. Health educators to address proportionality of health for the most disadvantaged and hard to reach communities in Brent. Apply here.
ACHIEVEMENTS AND RECOGNITIONS:
CENSUS 2021: Fantastic news, we are Census Champions.
We received an acknowledgment letter with census champion’s certificate for helping them as a result 97 of households completed their census form. We helped 101 households to complete their forms. Census Champion certificates and the letter of appreciation.
